New Freedom is introducing and launching a state-of-the-art behavioral health campus model concept of treatment programs in collaboration with Gold Canyon Heart & Home (GCHH), an AZ based 501(c)(3) nonprofit organization with a 17-year track record of hands on recovery & reentry programming. The end product is a fully cohesive continuum of complete behavioral health care treatment.
The inaugural campus location is in the heart of Central Phoenix where a 248 room hotel-resort was converted into the first of it's kind, a true behavioral health haven retreat. Here New Freedom provides a treatment environment unparalleled in the industry. Our the intention are to the client and experience of self-worth that all humans deserve.
Pre-Admission / Referral
Upon submitting your referral, a staff member from New Freedom will contact you to confirm that it has been received. After review, the client will be contacted to schedule an intake for services. Please be sure to fill out the below as completely and accurately as possible.
➔ Admission Criteria:
◆ Criminal justice involved meaning recent release from incarceration or on probation or parole
◆ Substance use within the last year or a general mental health diagnosis
◆ Ambulatory and able to complete activities of daily living and independent living skills (housing)
◆ Meets medical necessity for Intensive Outpatient Treatment
➔ New Freedom promotes a lifestyle free from MAT services. Please be aware we consider the following narcotic medications on a case by case evaluation. If an exception is made there will be a commitment of a titration plan for the member to be free from all MAT substances before graduating New Freedom programming:
◆ Opioid pain medication
◆ Medicinal Marijuana
◆ Benzodiazepine (Xanax, Klonopin, etc.)
◆ Narcotic sleeping medication (Ambien, etc.)
➔ If your referral client is taking the above medication, a documented titration plan must be included in the
referral packet signed by the prescribing physician.
➔ If your referral client has a history of mental health treatment, the mental health provider information must be included in the referral submission including assessment and treatment plan.
